Partners Home and Nursing Services offers Home Health Aide care that covers:

• General Symptom Monitoring
• Wound Care and Dressing Changes
• Vital Signs Monitoring
• Transportation Arrangements to Medical Appointments
• Safety Supervision
• Meal Preparation (According to person's Dietary Needs)
• Help with ADLs

Adult Foster Care (AFC)

The purpose of the Adult Foster Care program is to provide daily assistance with at least one Activities of Daily Living (such as bathing, dressing, toileting, transferring, mobility and eating) and house chores by a qualified caregiver to MassHealth members in the comfort of their homes. This service must be ordered by a primary care doctor. Services are delivered to a MassHealth member living in a qualified home setting.  We have a multidisciplinary team that includes AFC nursing oversight, and AFC care management.

Eligibility Criteria

Members/Clients must:

• Be sixteen (16) years or older.
• Be eligible for MassHealth coverage or other approved insurance payer.
• Have a medical or mental condition resulting in need for assistance with activities of daily living (ADLs) for at least one of the following: walking, eating, full body dressing, using the toilet, transfers (bed to chair).
• Be clinically approved by primary care physician (PCP) and MassHealth.
• Live in a qualified setting
