he Healthier You Adult Foster Care (HY- AFC) Program provides valuable caring help to adults 16 years and older. Because of illness, weakness, disability, or advanced age, they cannot live safely at home alone. This program is designed for those who do not require constant 24-hour monitoring but do need a significant amount of daily help.

Adult Foster Care programs are steeped in the fundamental belief that people have a right to remain at home, in a community setting. This is important to the success of our program. Choice leads to independence and dignity for the people, which helps foster a more meaningful and successful outcome.

As an AFC member, you will live with your trained Caregiver, who can be a family relative or an unrelated family friend. The Caregiver will provide help. They can prompt, or give help with:

• bathing
• dressing
• toileting
• transferring
• ambulating/walking
• eating

The Caregiver will also provide rides to medical appointments and be a good companion as well.
