Adult Foster Care provider

Adult Foster Care (AFC)
Adult Foster Care programs provide support to clients (people aged 16 or older who have a medical condition or disability that impacts daily functioning) and their in-home caregivers. Support includes tax-free stipends for caregivers and free monthly visits from a RN or CM, who will provide training specific to the person’s needs as well as support caregivers in providing the best possible daily care.

Group Adult Foster Care (GAFC)
GAFC programs provide Certified Personal Caregivers to assist with daily living activities to clients who are aged 22 or older, live in subsidized housing or assisted living facilities, and are elderly or disabled.
