Adult Foster Care

Welcome to Fortcom Adult Foster Care (AFC) Program. The AFC program serves  people who are 16 years of age or older. Members must have MassHealth Standard Insurance, and be diagnosed with a medical, behavioral or psychiatry condition that impairs abilities to perform basis daily life activities.

What Is Adult Foster Care (AFC)?

AFC Services is a MassHealth program that are ordered by a doctor. Services are delivered to a Member in a qualified home setting by a Qualified Caregiver. The Member will receive quality care at-home by a  team of Register Nurse services and oversight, AFC Provider Management, and qualified AFC Caregiver. Services include assistance with activities of daily living (ADLs), Instrumental Activities of Daily Living (IADLs), and other personal care as needed.
