Narrative

The mission of Unified Family & Youth Network (UFYN) is to provide high quality health care and support families in their home setting.  We will accomplish this mission through partnership with the people we serve, with a highly skilled team of diverse and culturally competent staff. The AFC program is designed to offer a living option for individuals with a medical or psychiatric diagnosis who require assistance with their activities of daily living (ADLs) in or der to maintain a safe lifestyle within the community.

Adult Family Care (AFC) is a program for frail elderly adults and adults with disabilities who cannot live alone safely. AFC adults live with trained paid caregivers who provide daily care.
