Narrative

For senior and disabled adults living in a GAFC approved setting, we provide the personal assistance services they need to remain an integrated and independent member of their community. Home health aides and licensed nurses deliver personal, medical, and nursing care with dignity and compassion. Our goal is to maintain our client’s health, safety, and independence to avoid long-term placements. GAFC includes physical assessment, case management, meal preparation, bathing & toileting, shopping & errands, laundry, nurse monitoring, medication reminders, and transportation. GAFC can visit within rest home & subsidized living as well.
